PMID: 35767897. Table 5 requires all 3 major criteria and at least 1 minor criterion confirmed in two consecutive determinations. Major criteria are the stage-specific marrow morphology, a JAK2, CALR, or MPL mutation or another clonal marker or absence of reactive fibrosis, and exclusion of other myeloid neoplasms. Minor criteria are anemia not from a comorbid condition, leukocytes ≥11×10^9/L, palpable splenomegaly, and LDH above the reference range. Overt PMF also counts leukoerythroblastosis. Prefibrotic morphology is megakaryocytic proliferation and atypia with fibrosis grade <2, increased age-adjusted cellularity, granulocytic proliferation, and often decreased erythropoiesis. Overt morphology is megakaryocytic proliferation and atypia with reticulin or collagen fibrosis grade 2 or 3. (2022)
