Pediatric SIRS, Sepsis, and Septic Shock calculator
Enter the published formula inputs below to calculate and interpret this score.
Clinical inputs
Allowed range: 86–109.4 °F
Abnormal when above 38.5°C or below 36°C. Rectal, bladder, oral, or central probe.
Allowed range: 0–300 beats/min
Tachycardia is above the Table 3 value. Bradycardia is below the printed value only in the three groups under 1 year.
Allowed range: 0–120 breaths/min
Tachypnea is above the Table 3 value.
Allowed range: 0–100000 cells/mm^3
Above or below the Table 3 values. The newborn row has only an upper value.
Allowed range: 0–100 %
Above 10% meets the leukocyte criterion.
Allowed range: 0–5 count
Count respiratory, renal, neurologic, hematologic, and hepatic items from Table 4. Two or more, with sepsis, is severe sepsis.

DOI: 10.1097/01.PCC.0000149131.72248.E6. PMID: 15636651. SIRS is at least two of four criteria, and one must be temperature or leukocyte count. Core temperature above 38.5°C or below 36°C; heart rate above the Table 3 95th percentile, or below the 5th percentile when that row prints a bradycardia value; respiratory rate above the Table 3 value or mechanical ventilation for an acute process; leukocyte count above or below the Table 3 values, or immature neutrophils above 10%. Sepsis is SIRS with suspected or proven infection. Septic shock is sepsis with cardiovascular dysfunction. Severe sepsis is sepsis with cardiovascular dysfunction, ARDS, or two or more other organ dysfunctions. (2005)
