Phoenix Sepsis Score calculator
Enter the published formula inputs below to calculate and interpret this score.
Clinical inputs
Allowed range: 0–18 years
Enter age in months. Eighteen years or older is not scored. Mean arterial pressure cuts change under 1 month, from 1 to 11 months, from 1 to under 2 years, from 2 to under 5 years, from 5 to under 12 years, and from 12 to 17 years.
Allowed range: 0–700 mm Hg
Leave blank if arterial oxygen tension was not measured. Divide by FiO2 entered as a fraction.
Allowed range: 21–100 %
Leave blank when no ratio is calculated. An unmeasured ratio adds no respiratory points.
Allowed range: 0–100 %
The saturation ratio is used only when SpO2 is 97 percent or lower.
Allowed range: 0–270.27 mg/dL
Leave blank if lactate was not measured. Below 5 mmol/L is 0, 5 through 10.9 is 1, and 11 or higher is 2.
Allowed range: 0–150 mm Hg
Leave blank if blood pressure was not measured. Prefer a measured mean. Otherwise use one third of systolic plus two thirds of diastolic.
Allowed range: 0–2000 10³/µL
Leave blank if not measured. Below 100 times 10 to the third per microliter adds 1 coagulation point.
Allowed range: 0.5–15
Leave blank if not measured. Above 1.3 adds 1 coagulation point.
Allowed range: 0–100000 ng/mL FEU
Leave blank if not measured. Above 2 mg/L FEU adds 1 coagulation point.
Allowed range: 0–1000 mg/dL
Leave blank if not measured. Below 100 mg/dL adds 1 coagulation point. Coagulation points stop at 2.
Allowed range: 3–15 points
A score of 10 or lower is 1 neurologic point unless both pupils are fixed.

Sanchez-Pinto LN et al. Development and validation of the Phoenix criteria for pediatric sepsis and septic shock. JAMA. DOI: 10.1001/jama.2024.0196. PMID: 38245897. Table 2 scores respiratory 0–3 from PaO2:FiO2 or SpO2:FiO2 (SpO2:FiO2 only when SpO2 is 97% or lower; 2 or 3 points require invasive ventilation; 1 point requires any respiratory support, including a ratio below the 2-point cut in a child who is not invasively ventilated), cardiovascular 0–6 as the sum of vasoactive medicines (0, 1, or at least 2), lactate (<5, 5–10.9, or at least 11 mmol/L), and age-specific mean arterial pressure, coagulation 0–2 with 1 point each (maximum 2) for platelets <100×10³/µL, INR >1.3, D-dimer >2 mg/L FEU, or fibrinogen <100 mg/dL, and neurologic 0–2 (Glasgow coma score 10 or lower is 1; bilaterally fixed pupils are 2). Unmeasured items add no points. In suspected infection, 2 or more points is sepsis, and sepsis with at least 1 cardiovascular point is septic shock. The criteria do not apply at age 18 years or older. (2024)
