%

Allowed range: 21–100 %

Quick picks

Enter FiO2 as a fraction from 0.21 to 1. Respiratory points use saturation divided by that fraction.

Cardiovascular row

Choose the single highest cardiovascular row from the modified score table.

points

Allowed range: 3–15 points

Quick picks
mg/dL

Allowed range: 0.1–30 mg/dL

Quick picks

A creatinine above 4.9 mg/dL and not above 5.0 mg/dL is not on a printed row and is not scored.

Grissom CK et al. A modified sequential organ failure assessment score for critical care triage. Disaster Med Public Health Prep. DOI: 10.1001/dmp.2010.40. PMID: 21149228. Table 2: SpO2/FiO2 above 400 is 0, 400 or lower is 1, 315 or lower is 2, 235 or lower is 3, and 150 or lower is 4. Jaundice or scleral icterus is 3 and its absence is 0. Cardiovascular rows match the SOFA rows in the same paper: no hypotension 0, mean arterial pressure below 70 mm Hg 1, dopamine 5 or lower or any dobutamine 2, dopamine above 5 or epinephrine or norepinephrine 0.1 or lower 3, and dopamine above 15 or epinephrine or norepinephrine above 0.1 is 4, with doses in µg/kg/min as in that SOFA table. Glasgow coma score 15 is 0, 13–14 is 1, 10–12 is 2, 6–9 is 3, and below 6 is 4. Creatinine below 1.2 mg/dL is 0, 1.2–1.9 is 1, 2.0–3.4 is 2, 3.5–4.9 is 3, and above 5.0 is 4. A creatinine above 4.9 and not above 5.0 is not on a printed row. The paper reports 58% 30-day mortality when the score is above 11. (2010)
