RADS (Radiologist’s Score) for Smoke Inhalation Injury calculator
Enter the published formula inputs below to calculate and interpret this score.
Clinical inputs
Allowed range: 0–2400 points
On each 1-cm CT slice, split each lung into 4 quadrants (8 per slice). Score each quadrant by its worst finding: normal 0, increased interstitial markings 1, ground-glass opacity 2, consolidation 3. Add all quadrants on all slices.
Allowed range: 1–100 slices
Slices from the lung apex to the diaphragm.

Oh JS, Chung KK, Allen A, Batchinsky AI, Huzar T, King BT, Wolf SE, Sjulin T, Cancio LC. Admission chest CT complements fiberoptic bronchoscopy in prediction of adverse outcomes in thermally injured patients. J Burn Care Res. 2012;33(4):532-538. DOI: 10.1097/BCR.0b013e318237455f. PMID: 22210063. Methods: admission chest CT is read in 1-cm axial slices from apex to diaphragm; each lung field on each slice is divided into four quadrants (eight per slice); each quadrant scores its worst finding: normal 0, increased interstitial markings 1, ground-glass opacification 2, consolidation 3. Quadrant scores are summed per slice and across all slices for the total RADS, and the total divided by the number of slices read gives RADS per slice (maximum 24). RADS per slice was higher with inhalation injury (median 7.1 vs 3.0). The composite outcome was death, ALI/ARDS and/or pneumonia. RADS per slice above 8 alone was not significantly associated with the composite outcome (Table 5: present in 36% of patients with vs 18% without the composite outcome, P = .18); inhalation injury plus RADS per slice above 8 had an odds ratio of 12.7 (95% CI 1.1-153.7) in the 44-patient cohort. The text defines the binary cut as above 8 vs 8 or less (one table row is labelled 8 or more; the text definition is used here). (2012)
