Primary source

Brahmer JR et al. Management of Immune-Related Adverse Events in Patients Treated With Immune Checkpoint Inhibitor Therapy: American Society of Clinical Oncology Clinical Practice Guideline. J Clin Oncol. PMID: 29442540. Table 6, section 6.1 Nephritis. Definition: inflammation of the kidney affecting the structure. Grade 1: creatinine increase of more than 0.3 mg/dL, or creatinine 1.5 to 2.0 times baseline. Grade 2: creatinine 2 to 3 times baseline. Grade 3: creatinine more than 3 times baseline or more than 4.0 mg/dL, or hospitalization indicated. Grade 4: life-threatening consequences, or dialysis indicated. A ratio of exactly 2 is inside both the grade 1 and grade 2 rows, so the higher grade is returned. A ratio of exactly 3 stays in grade 2 because grade 3 requires more than 3 times baseline. A creatinine of exactly 4.0 mg/dL is not more than 4.0 mg/dL. When more than one printed finding is present, the higher grade is returned. Management text from the table is not used. µmol/L entries are divided by 88.4 to reach mg/dL. (2018)
