Primary source

Edmiaston J et al. A simple bedside stroke dysphagia screen, validated against videofluoroscopy, detects dysphagia and aspiration with high sensitivity. J Stroke Cerebrovasc Dis. DOI: 10.1016/j.jstrokecerebrovasdis.2013.06.030. PMID: 23910514. The screen fails when any of five items is abnormal: Glasgow Coma Scale below 13, facial asymmetry or weakness, tongue asymmetry or weakness, palatal asymmetry or weakness, or signs of aspiration on the 3-ounce water swallow. A pass requires all five to be normal. Against videofluoroscopy, sensitivity for dysphagia was 94% and for aspiration 95%. The same fail-if-any rule is in Edmiaston J et al. Am J Crit Care. 2010. PMID: 19875722. (2014)
