Primary source
Crotalidae polyvalent immune Fab (ovine), CROFAB, prescribing information, FDA. Section 2.1. The recommended initial dose is 4 to 6 vials. The starting dose may vary from a minimum of 4 vials to a maximum of 12 vials. If initial control is not achieved by the first dose, an additional dose of 4 to 6 vials should be administered repeatedly until initial control has been achieved. Once initial control has been achieved, additional 2-vial doses every 6 hours for up to 18 hours (3 doses) are recommended. The label does not print a maximum number of those repeat doses or a maximum course total, so repeats after the first extra dose are not added. When control is already achieved, the result is the initial vials plus 6 maintenance vials. When control is not achieved, the result is the initial vials plus one extra dose of 4 to 6 vials, and the 6 maintenance vials stay out of the result until control. Section 2.1 does not print a per-kilogram dose or a kilogram cap. The 250 kg entry limit is not a label cap, and weight does not change the vial count. The dose has to be verified against the local protocol. (2018)
