Primary source
St-Onge M et al. Experts Consensus Recommendations for the Management of Calcium Channel Blocker Poisoning in Adults. Crit Care Med. PMID: 27749343. The proposed regimen of regular insulin is a bolus of 1 unit/kg followed by an infusion of 1 unit/kg/h, with euglycemia maintained by a dextrose infusion as needed. Titration up to 10 units/kg/h is the printed upper rate for patients who do not respond to first-line therapies or who have refractory shock. The result is the 1 unit/kg/h infusion in units/h. The bolus and the 10 units/kg/h ceiling are separate lines. The paper does not print a kilogram cap or a fixed dextrose rate, so neither was added. The 30 kg and 250 kg entry limits are not printed caps. The dose has to be verified against the local protocol. (2017)
