- Active cancer
- Paralysis, paresis, or recent plaster immobilization
- Bedridden >3 days or major surgery within 4 weeks
- Localized tenderness along the deep venous system
- Entire leg swelling
- Calf swelling >3 cm vs other leg
- Pitting edema confined to the symptomatic leg
- Collateral superficial veins (non-varicose)
- Alternative diagnosis at least as likely as DVT
