Primary source

Goldfrank L, Weisman RS, Errick JK, Lo MW. A dosing nomogram for continuous infusion intravenous naloxone. Ann Emerg Med. PMID: 3963538. A continuous infusion of two-thirds of the bolus dose that resulted in reversal, each hour, maintained plasma levels at or above the level 30 minutes after that bolus. The result is that hourly rate in mg/h. The paper does not print an infusion maximum or a kilogram cap, and the rate does not use weight. The 100 mg and 250 kg entry limits are not printed caps. Naloxone hydrochloride injection labeling states that if no response is observed after 10 mg, the diagnosis of opioid-induced toxicity should be questioned. That 10 mg figure is not an infusion maximum and does not change the hourly rate. The dose has to be verified against the local protocol. (1986)
