Antzelevitch C et al. J-wave syndromes expert consensus conference report: emerging concepts and gaps in knowledge. Heart Rhythm. DOI: 10.1016/j.hrthm.2016.05.024. PMID: 27423412. Table 2, proposed Shanghai score. ECG, counted once at the highest applicable item and required: spontaneous type 1 pattern 3.5, fever-induced type 1 pattern 3, type 2 or 3 pattern that converts with a drug challenge 2. Clinical history, highest item only: unexplained arrest or documented VF or polymorphic VT 3, nocturnal agonal respirations 2, suspected arrhythmic syncope 2, syncope of unclear mechanism 1, atrial flutter or fibrillation under age 30 without another cause 0.5. Family history, highest item only: first- or second-degree relative with definite Brugada syndrome 2, suspicious sudden death in a first- or second-degree relative 1, unexplained sudden death under 45 years with a negative autopsy in a first- or second-degree relative 0.5. A probable pathogenic mutation in a Brugada susceptibility gene is 0.5. At least 3.5 is probable or definite, 2 through 3 is possible, and under 2 is nondiagnostic. Without an ECG item the score is nondiagnostic. (2016)
