Margin and surface: zero or diffuse 0; sharp but irregular, jagged, geographical, or satellites 1; sharp and even, with a difference in surface level including cuffing 2. Vessels: fine and regular 0, absent 1, coarse or atypical 2. Lesion size: <5 mm 0, 5–15 mm or two quadrants 1, >15 mm or three to four quadrants or endocervically undefined 2. Iodine: brown 0, faintly or patchy yellow 1, distinct yellow 2. A total under 5 reasonably excludes a high-grade lesion and biopsy may be avoided. A score of 5 to 7 is seen with low-grade or high-grade lesions and biopsy is indicated. A score exceeding 7 indicates a high-grade lesion or invasive cancer. (2022)
