Chuan F, Tang K, Jiang P, Zhou B, He X. Reliability and validity of the perfusion, extent, depth, infection and sensation (PEDIS) classification system and score in patients with diabetic foot ulcer. PLoS One. DOI: 10.1371/journal.pone.0124739. PMID: 25875097. Table 1 assigns the Score column to each grade: grade 1 scores 0, grade 2 scores 1, grade 3 scores 2, and grade 4 scores 3. Perfusion: no peripheral arterial disease 0, peripheral arterial disease without critical limb ischemia 1, critical limb ischemia 2. Extent: skin intact 0, under 1 cm² 1, 1–3 cm² 2, over 3 cm² 3. Depth: skin intact 0, superficial 1, fascia muscle or tendon 2, bone or joint 3. Infection: none 0, surface 1, abscess fasciitis or septic arthritis 2, systemic inflammatory response 3. Sensation: no loss 0, loss 1. The five categories sum to a maximum of 12. The receiver-operating-characteristic threshold for adverse outcome was 7. Scores of 7 or higher are placed on the adverse side of that printed threshold, and scores of 6 or lower are not. Extent uses the largest diameter times the perpendicular diameter. Intact skin scores 0 for extent even if a number is entered. Area under 1 cm² scores 1, 1 cm² through 3 cm² scores 2, and above 3 cm² scores 3. (2015)
