PMID: 22153173, walks one paper score: age 55 to 64 years is 13, male is 6, and one first-degree category is 2. Age 40 to 44 years is 0, 45 to 54 is 7, 55 to 64 is 13, and 65 to 74 is 15. Male is 6 and female is 0. Body mass index less than 25 is 0, 25 to 29 is 4, 30 to 34 is 9, and 35 and over is 14. The worked chart example of about 29.8 kg/m² is in the 25-to-29 group, so 25 up to but not including 30 is 4 points and 30 up to but not including 35 is 9. Waist in men: less than 94 cm is 0, 94 to 102 cm is 4, and over 102 cm is 6. Waist in women: less than 80 cm is 0, 80 to 88 cm is 4, and over 88 cm is 6. High blood sugar on a test, during illness, or in pregnancy is 14. A baby of 9 pounds (4.1 kg) or more is 1, and no, unknown, or not applicable is 0. Mother, father, brothers or sisters, and children each add 2, with a combined maximum of 8. Other relatives and unknown family history add 0. Parent ethnicity uses the higher of the two parents and does not add them: White 0, Aboriginal 3, Black 5, East Asian 10, South Asian 11, other non-white 3, and that section cannot exceed 11. Education: some high school or less is 5, a high school diploma is 1, and some college or university or a degree is 0. Lower than 21 is low risk, 21 to 32 is moderate risk, and 33 and over is high risk. The validation text gives a maximum of 86 for men and 81 for women when men score 0 on the birth question. (2011)
