Total dose 300 mg/kg over 21 hours: bag 1 is 150 mg/kg over 1 hour, bag 2 is 50 mg/kg over 4 hours, and bag 3 is 100 mg/kg over 16 hours. At 100 kg or greater the bags are 15,000 mg, 5,000 mg, and 10,000 mg. Diluent volumes are 3, 7, and 14 mL/kg at 5–20 kg; 100, 250, and 500 mL at 21–40 kg; and 200, 500, and 1,000 mL at 41 kg or greater.
