Clinical inputs

Airway intubation

Intubation for myasthenic weakness is class V. Routine postoperative ventilation does not assign class V.

Weakness of muscles other than the eyes

Class I requires normal strength outside the eyes, including eye closure that may be weak without changing the class. Mild, moderate, and severe describe classes II, III, and IV.

Jaretzki A et al. Myasthenia gravis: recommendations for clinical research standards. Neurology. DOI: 10.1212/wnl.55.1.16. PMID: 10891897. Class I is ocular weakness with all other muscle strength normal. Classes II, III, and IV are mild, moderate, and severe weakness of non-ocular muscles; subclass a is predominantly limb or axial and subclass b is predominantly oropharyngeal or respiratory. Class V is intubation for myasthenic weakness, excluding routine postoperative ventilation. A feeding tube without intubation is class IVb. The classification is not an outcome measure. (2000)
