Agent

Lidocaine uses the adult individual maximum from section 2.6. Bupivacaine uses the Table 3 each-dose upper bound for local infiltration and for peripheral nerve block. Caudal, epidural, retrobulbar, sympathetic, and dental rows are not included.

Epinephrine
lb

Allowed range: 88.18–551.16 lb

Quick picks

Body weight in kilograms. Lidocaine is 7 mg/kg with epinephrine, absolute maximum 500 mg, or 4.5 mg/kg without epinephrine, absolute maximum 300 mg. The opened bupivacaine label does not print a mg/kg maximum, so this weight does not change that result.

Primary source

Lidocaine hydrochloride injection prescribing information, FDA application 006488, revised October 2024, section 2.6 Adults. For normal healthy adults the individual maximum of lidocaine hydrochloride with epinephrine should not exceed 7 mg/kg of body weight, and in general the maximum total dose should not exceed 500 mg. Without epinephrine the maximum individual dose should not exceed 4.5 mg/kg of body weight, and in general the maximum total dose should not exceed 300 mg. The result is the lower of those two amounts. The label uses body weight and does not print a kilogram cap. Intravenous regional anesthesia (4 mg/kg) and paracervical block (200 mg per 90 minutes) are separate printed limits and are not this result. Bupivacaine hydrochloride injection and bupivacaine hydrochloride and epinephrine injection prescribing information, FDA application 018053, label 018053s061. Table 3 prints an each-dose upper bound of up to 175 mg without epinephrine and up to 225 mg with epinephrine for local infiltration and for peripheral nerve block. The same label says these doses may be repeated once every three hours and not to exceed a total daily dosage of 400 mg in 24 hours. That label does not print a mg/kg maximum, so body weight does not change the bupivacaine milligrams. Other Table 3 rows have lower upper bounds and are not this result. The dose has to be verified against the local protocol. (2024)
