Primary source
Kruis W et al. A diagnostic score for the irritable bowel syndrome. Its value in the exclusion of organic disease. Gastroenterology. 1984;87:1-7. PMID: 6724251. Figure 1 patient points: +34 if the visit was for abdominal pain, flatulence, or irregular bowel movement; +16 if complaints lasted more than 2 years; +23 if pain is described as burning, cutting, very strong, terrible, pressure, dull, boring, or not so bad, and only when at least one first-line statement is yes or more than two patient statements in total are yes; +14 for alternating constipation and diarrhea. Stool form, fever above 38.5°C, underweight, and loss of more than 5 kg in 6 months are printed with a dash and are not added. Doctor points: abnormal findings pathognomonic for another diagnosis -47; ESR above 20 mm/2 h -13; leukocyte count above 10000/mm³ -50; hemoglobin under 12 g/dL in a woman or under 14 g/dL in a man -98; history of blood in the stool -98. A score of 44 or higher was the minimum that positively diagnosed irritable bowel syndrome and excluded organic disease at 99% specificity and 64% sensitivity. At the study prevalence of about 20%, the positive predictive value at that score was 94%. (1984)
