Irritable bowel syndrome is at least 12 weeks, which need not be consecutive, in the preceding 12 months, of abdominal discomfort or pain with two of three features: relieved with defecation; onset associated with a change in frequency of stool; onset associated with a change in form of stool. The diagnosis presumes no structural or biochemical explanation. Supportive symptoms are not required: fewer than 3 bowel movements a week or more than 3 a day; hard or lumpy stools or loose or watery stools; straining, urgency, or incomplete evacuation; mucus; and abdominal fullness, bloating, or swelling.
