EUROMACS-RHF Score calculator

Enter the published formula inputs below to calculate and interpret this score.

Clinical inputs

Allowed range: 0–3

Quick picks

A ratio above 0.54 scores 2 points. A ratio of 0.54 does not.

g/dL

Allowed range: 4–20 g/dL

Quick picks

10 g/dL or lower scores 1 point. 10.1 g/dL does not. 100 g/L is the same threshold.

Three or more intravenous inotropes

The text defines multiple intravenous inotropes as 3 or more. That row scores 2.5 points.

INTERMACS class 1, 2, or 3

Classes 1 through 3 score 2 points. Classes 4 through 7 do not.

Severe right ventricular dysfunction

Semiquantitative echocardiographic assessment of right ventricular systolic function. Severe dysfunction scores 2 points.

Primary source

Soliman OII et al. Derivation and validation of a novel right-sided heart failure model after implantation of continuous flow left ventricular assist devices: the EUROMACS right-sided heart failure risk score. Circulation. DOI: 10.1161/CIRCULATIONAHA.117.030543. PMID: 28847897. Table 6 preoperative model: right atrial to pulmonary capillary wedge pressure ratio above 0.54 scores 2; hemoglobin 10 g/dL or lower scores 1; multiple intravenous inotropes, defined in the text as 3 or more, scores 2.5; INTERMACS class 1–3 scores 2; severe right ventricular dysfunction on echocardiography scores 2. The printed example is INTERMACS class 3, severe right ventricular dysfunction, ratio 0.55, three inotropes, and hemoglobin 10 g/dL: 2 + 1 + 2.5 + 2 + 2 = 9.5. Low risk is 0–2 (11% early right-sided heart failure in the derivation cohort). High risk is above 4 (43.1%). The remaining scores, above 2 through 4, are the intermediate category named in the same results. The postoperative model in the same table uses different weights and adds cardiopulmonary bypass time, and it is not this score. (2018)
