Bösel J, Schiller P, Hacke W, Steiner T. Benefits of early tracheostomy in ventilated stroke patients? Current evidence and study protocol of the randomized pilot trial SETPOINT. Int J Stroke. DOI: 10.1111/j.1747-4949.2011.00703.x. PMID: 22264372. Table 3 points: dysphagia 4, observed aspiration 3, admission Glasgow coma score under 10 scores 3, brainstem 4, space-occupying cerebellar lesion 3, ischemic infarct greater than two thirds of the middle-cerebral-artery territory 4, intracerebral hemorrhage volume greater than 25 mL scores 4, diffuse lesion 3, hydrocephalus 4, neurosurgical intervention 2, additional respiratory disease 3, PaO2/FiO2 under 150 scores 2, acute physiology score of APACHE II greater than 20 scores 4, lung injury score greater than 1 scores 2, and sepsis 3. That table estimates at least two weeks of ventilation when the sum is greater than 10. Schönenberger S et al., Neurocrit Care, PMID: 26842719, later reported a score value of 8 as the ROC operating point for prolonged neurointensive stay, ventilation, and tracheostomy. A sum of 7 stays below that operating point. A sum of 8 through 10 meets it and does not pass the Table 3 threshold of greater than 10. A sum of 11 or more passes both. (2012)
