Vaid S, Bell T, Grim R, Ahuja V. Predicting risk of death in general surgery patients on the basis of preoperative variables using American College of Surgeons National Surgical Quality Improvement Program data. Perm J. DOI: 10.7812/TPP/12-019. PMID: 23251111. Table 5, a 30-point bedside scoring system: inpatient 6, sepsis 4, poor functional status 3, disseminated cancer 1, age at least 80 years 2, age 70–79 years 1, age 65–69 years 0.5, cardiac comorbidity 5, pulmonary comorbidity 3, renal comorbidity 1, liver comorbidity 1, steroid for a chronic condition 1, weight loss 1, bleeding disorder 1, do-not-resuscitate directive 1, obesity −1. Under 65 years scores 0. The printed range is −1 through 30 and the highest possible score is 30. The discussion sentence that age points rise from 1 to 1.5 to 2.5 is not used, because those points would pass the printed maximum of 30. Weight loss is greater than 10% of body weight in the last 6 months, so 10% scores 0 and more than 10% scores 1. Methods definitions: pulmonary is ventilator dependence, a history of chronic obstructive pulmonary disease, or current pneumonia; liver is ascites or esophageal varices; renal is dialysis or renal failure; cardiac is dyspnea or a history of congestive heart failure, myocardial infarction, angina, peripheral artery disease, or hypertension requiring medication; poor functional status is total assistance for every activity of daily living (the totally dependent NSQIP level). Figure 3 groups scores as −1 to 5, 6 to 10, 11 to 15, 16 to 20, 21 to 25, and 26 to 30, and prints the proportion who died as 0.001, 0.006, 0.024, 0.126, 0.375, and 0.4. A total ending in 0.5 stays in the lower bin because the next printed bin starts at the next integer. (2012)
