The revised tool keeps nine items and scores each yes as 1, after dropping preadolescent sexual abuse. The items are family history of alcohol, illegal drugs, and prescription drugs; personal history of alcohol, illegal drugs, and prescription drugs; age 16–45 years; attention deficit disorder, obsessive compulsive disorder, bipolar disorder, or schizophrenia; and depression.
