Opioid in the 2022 table

Fentanyl transdermal uses the mcg/h field. The other nine opioids use the mg/day field. The field that does not apply does not change the result.

mg/day

Allowed range: 0.1–2000 mg/day

Quick picks

Used for the nine opioids that are not fentanyl. This field does not change a fentanyl result. If one of those nine is selected and this dose is missing, the mcg/h field is not used. The table prints no maximum dose. The 2000 mg/day entry limit is not a table cap.

mcg/h

Allowed range: 0.1–2000 mcg/h

Quick picks

Used only when the opioid is fentanyl transdermal. Enter mcg/h. The factor is 2.4 MME per mcg/h. This field does not change any other opioid. If fentanyl is selected and this dose is missing, the mg/day field is not used. The 2000 mcg/h entry limit is not a table cap.

lb

Allowed range: 66.14–551.16 lb

Quick picks

The table does not use weight and does not print a kilogram cap. The 250 kg entry limit is not a table cap.

Dowell D, Ragan KR, Jones CM, Baldwin GT, Chou R. CDC Clinical Practice Guideline for Prescribing Opioids for Pain — United States, 2022. MMWR Recomm Rep. PMID: 36327391. Table. Morphine milligram equivalent doses for commonly prescribed opioids. Multiply the dose by the conversion factor. Factors: codeine 0.15, fentanyl transdermal in mcg/h 2.4, hydrocodone 1.0, hydromorphone 5.0, methadone 4.7, morphine 1.0, oxycodone 1.5, oxymorphone 3.0, tapentadol 0.4, tramadol 0.2. Fentanyl transdermal is entered in mcg/h. The other nine opioids are entered in mg/day. The unused dose does not change the result. If the selected opioid's dose is missing, the other field is not used. Worked table examples: hydrocodone 20 mg/day is 20 MME/day, oxycodone 20 mg/day is 30 MME/day, and fentanyl transdermal 25 mcg/h is 60 MME/day. The table says not to use the calculated MME dose as the dose of a different opioid. It prints one methadone factor, 4.7, and does not print a hard maximum daily dose. The 50 MME/day figure is the point at which clinicians should pause before a dosage increase. Weight is not in the table and no kilogram cap is printed. The 2000 mg/day, 2000 mcg/h, and 250 kg entry limits are not table caps. The dose has to be verified against the local protocol. (2022)
