Below 45 years is 0, 45 up to 55 is 1, 55 up to 65 is 2, 65 up to 75 is 3, and 75 years or older is 4.
Allowed range: 8–50 mm Hg
Use the mean of the two eyes.
Allowed range: 400–800 µm
Use the mean of the two eyes.
Allowed range: 0–1 ratio
Below 0.3 is 0, 0.3 up to 0.4 is 1, 0.4 up to 0.5 is 2, 0.5 up to 0.6 is 3, and 0.6 or higher is 4. Use the mean of the two eyes.

Validated prediction model for the development of primary open-angle glaucoma in individuals with ocular hypertension. Ophthalmology. DOI: 10.1016/j.ophtha.2006.06.061. PMID: 17095090. Table 6 assigns 0 to 4 points. Age below 45 years is 0, 45 up to 55 is 1, 55 up to 65 is 2, 65 up to 75 is 3, and 75 or older is 4. Mean intraocular pressure below 22 mm Hg is 0, 22 up to 24 is 1, 24 up to 26 is 2, 26 up to 28 is 3, and 28 or higher is 4. Mean central corneal thickness of 600 µm or more is 0, 576 up to 600 is 1, 551 to 575 is 2, 526 to 550 is 3, and 525 µm or less is 4. A thickness of 600 µm uses the 0-point column. Mean vertical cup-to-disc ratio below 0.3 is 0, 0.3 up to 0.4 is 1, 0.4 up to 0.5 is 2, 0.5 up to 0.6 is 3, and 0.6 or higher is 4. Mean pattern standard deviation below 1.8 dB is 0, 1.8 up to 2.0 is 1, 2.0 up to 2.4 is 2, 2.4 up to 2.8 is 3, and 2.8 or higher is 4. Sums of 0 to 6, 7 to 8, 9 to 10, 11 to 12, and above 12 estimate a 5-year risk of at most 4.0 percent, 10 percent, 15 percent, 20 percent, and at least 33 percent. The printed example is a 55-year-old with mean intraocular pressure 24 mm Hg, mean thickness 540 µm, mean cup-to-disc ratio 0.4, and mean pattern standard deviation 2.2 dB, totaling 11 points and a 20 percent estimate. Eye-specific inputs are the mean of the two eyes. Diabetes and race are not in this point system. (2007)
