Dowell D, Ragan KR, Jones CM, Baldwin GT, Chou R. CDC Clinical Practice Guideline for Prescribing Opioids for Pain — United States, 2022. MMWR Recomm Rep. PMID: 36327391. Milligram morphine equivalents equal the daily dose times the CDC conversion factor (codeine 0.15, fentanyl patch 2.4 per mcg/hour, hydrocodone 1, hydromorphone 4, morphine 1, oxycodone 1.5, oxymorphone 3, tapentadol 0.4, tramadol 0.1). Methadone uses the dose-banded factors 4, 8, 10, and 12. The guideline advises a pause before increasing the total to 50 MME/day or more. For a fentanyl patch, enter the strength in mcg/hour and 1 dose per day. (2022)
