CARG Chemotherapy Toxicity Tool calculator
Enter the published formula inputs below to calculate and interpret this score.
Clinical inputs
Allowed range: 18–110 years
The cohort started at age 65. Under 65 the score is not calculated. Age 72 or older scores 2.
Allowed range: 4–20 g/dL
Three points when hemoglobin is below 11 g/dL in men or below 10 g/dL in women. The cutoff value itself does not score.
Allowed range: 1–200 mL/min
Use the Jelliffe clearance with ideal weight. Below 34 mL/min scores 3. A value of exactly 34 does not.

Hurria A et al. Predicting chemotherapy toxicity in older adults with cancer: a prospective multicenter study. J Clin Oncol. DOI: 10.1200/JCO.2011.34.7625. PMID: 21810685. Table 5 gives 2 points for age ≥72 years, GI or GU cancer, standard-dose chemotherapy, polychemotherapy, hearing fair or worse, and limited walking one block; 3 points for hemoglobin <11 g/dL in men or <10 g/dL in women, Jelliffe ideal-weight creatinine clearance <34 mL/min, and one or more falls in 6 months; and 1 point for needing help with medicines and for social activity limited at least sometimes by health. Low risk is 0–5 points (30% grade 3–5 toxicity), intermediate is 6–9 (52%), and high is 10 or more (83% in the published 10–19 stratum). (2011)
