Structured clinical note formats

Adaptive style memory

What if a patient misinterprets their summary?
Every summary includes clear plain-language guidance explaining what it is and what it is not — including that it is not a final diagnosis, not a formal record, and that the patient must always follow their doctor's advice and contact emergency services if their condition changes. The language is deliberately warm and accessible — not alarming — but the limitations are stated clearly. The 7-day expiry also means outdated summaries cannot be accessed or misused long after the consultation.
