Formula inputs
- Hemodynamically unstable
- Thrombolysis or embolectomy necessary
- Active bleeding or high bleeding risk
- Needs oxygen for >24 hours to maintain saturation >90%
- Pulmonary embolism diagnosed during anticoagulant treatment
- Severe pain requiring IV analgesia for >24 hours
- Medical or social reason for hospital admission
- Creatinine clearance <30 mL/min
- Severe liver impairment
- Pregnant
- Documented history of heparin-induced thrombocytopenia
