Formula inputs

  • Hemodynamically unstable
  • Thrombolysis or embolectomy necessary
  • Active bleeding or high bleeding risk
  • Needs oxygen for >24 hours to maintain saturation >90%
  • Pulmonary embolism diagnosed during anticoagulant treatment
  • Severe pain requiring IV analgesia for >24 hours
  • Medical or social reason for hospital admission
  • Creatinine clearance <30 mL/min
  • Severe liver impairment
  • Pregnant
  • Documented history of heparin-induced thrombocytopenia
